Welcome to Our Practice
If you are 18 years or older, your medical records are regarded as personal and confidential, according to government HIPAA regulations. They cannot be disclosed without your written consent. Dr. Michael D. John and Dr. Andrew R. John at the Edmond Dermatology Clinic can provide you the HIPAA guidelines as well as “Designation of Personal Representative” forms.
Financial Policies
In an effort to reduce costs, both to our office and to you, we ask you to pay all appropriate charges the day of your appointment.
We are happy to file your insurance claim for reimbursement. Insurance co-payments are due at check-in on the date of service. When making your appointment, please let the receptionist know if your insurance requires a referral from your primary care physician. If so, referrals must be received prior to your appointment.
All primary and secondary insurances are filed within 24-72 hours for your convenience.
Medicare Provider
Dr. Michael D. John and Dr. Andrew R. John are Medicare providers and accept Medicare assignments. If you are a Medicare recipient, after you meet your deductible, Medicare will pay 80% and you will be responsible for 20% of the bill unless we are filing a secondary insurance for you. We ask that you pay your part of those charges the day of your appointment.
Fill Out Patient Information Form Before Your First Visit
Save time by downloading and completing our Patient Information Form. Bring it along with your insurance card, photo ID, name of pharmacy, phone number and list of allergies, as well as a complete list of all medications you take, over-the-counter and prescribed. To avoid charges for missed appointments, please call 24 hours in advance to cancel.
