For anyone 18 years or older, your medical records are regarded as personal and confidential, according to government HIPAA regulations. They cannot be disclosed without your written consent. Edmond Dermatology Clinic can provide you the HIPAA guidelines as well as "Designation of personal representative" forms.
In an effort to reduce costs, both to our office and for our patients, we ask you to pay all appropriate charges the day of your appointment.
We'll be happy to file your insurance company claim for reimbursement and insurance co-payments are due at check-in on the date of service. When making your appointment, please let the receptionist know if your insurance requires a referral from your primary care physician. If so, referrals must be received prior to your appointment.
All primary and secondary insurances are filed within 24-72 hours for your convenience.
Edmond Dermatology Clinic is a Medicare provider and accepts Medicare assignments. If you are a Medicare recipient, after you meet your deductible, Medicare will pay 80% and you will be responsible for 20% of the bill unless we are filing a secondary insurance for you. We ask that you pay your part of those charges the day of your appointment.
